Submit a certificate of fitness to drive

You'll need to complete a certificate of fitness if:

  • you have a medical condition that has been recorded and is subject to periodic review
  • you are 70 or older and have a licence class other than car - eg heavy vehicle or motorbike.

If you are required to do this a certificate of fitness form will be posted to you.

1. Complete the form

Fill in the Section 1 of the form you received in the post, or download the one that applies to your class of licence:

Your health professional must complete the rest of the form. For some medical conditions you'll need a specialist letter.

2. Submit the form

The form must be returned to Service SA by the due date.
