You'll need to complete a certificate of fitness if:
- you have a medical condition that has been recorded and is subject to periodic review
- you are 70 or older and have a licence class other than car - eg heavy vehicle or motorbike.
If you are required to do this a certificate of fitness form will be posted to you.
1. Complete the form
Fill in Section 1 of the form you received in the post, or download the one that applies to your class of licence:
- MR712 Certificate of fitness for light vehicles (72.8 KB PDF)
- MR713 Certificate of fitness for heavy vehicle and commercial drivers (129.5 KB PDF)
Your health professional must complete the rest of the form. For some medical conditions, you'll need a specialist letter.
2. Submit the form
The form must be returned to Service SA by the due date.
Scan or use a mobile phone to photograph all pages and email to servicesa@sa.gov.au.

Submit in person at a Service SA centre.
Post

What happens next
Once you've submitted your documents, you may continue to drive unless advised otherwise by your doctor or Service SA.
Our officers will review your documents. We will contact you if we need more information.
