Submit your certificate of fitness to drive

You'll need to complete a certificate of fitness if you:

  • have a medical condition that has been reported and is subject to periodic review
  • are 70 or older and have a licence class other than car — such as heavy vehicle or motorbike — unless you decide to downgrade your licence
  • hold a driver accreditation.

If you are required to do this a certificate of fitness form will be posted to you.

  1. Step 1

    Complete your section on the form

    You'll need to fill out Section 1 of the form you received in the post, or download and print one.

    For licence classes Car (C), Light Rigid (LR), Restricted Motorbike (R-Date) and Motorbike (R):

    For licence classes Medium Rigid (MR), Heavy Rigid (HR), Heavy Combination (HC), Multi Combination (MC), and any type of driver accreditation:

  2. Step 2

    Take your form to a health practitioner

    Your health practitioner must complete the rest of the form.

    For some medical conditions, you'll need a specialist letter.

  3. Step 3

    Submit the form

    The form must be returned to Service SA by the due date.

    In person

    Submit the form in person at a Service SA centre.

    Post

    Mail the completed form to:

    Adelaide SA 5001

What happens next

Once you've submitted your form, you may continue to drive unless advised otherwise by your doctor or Service SA.
