Holders of a car licence who are aged over 75 and have not already had a medical condition recorded, must complete an annual self assessment of their fitness to drive. A notification is sent around the time of the licence holder’s birthday.
Those who have already had a medical condition recorded must submit a certificate of fitness completed by a medical practitioner.

Using the form
Fill in Part A of the form you received in the post or download a copy - Medical fitness self assessment form.
If you answer 'yes' or 'unsure' to any question, you must take the form to a medical practitioner and have them complete the rest of the form.
In person
Submit your self assessment in person at a Service SA centre.

You can complete your self-assessment over the phone by calling 13 10 84 Monday to Friday between 9:00 am to 5:00 pm (excluding public holidays).
If you have a medical condition to declare or answer 'yes' or 'unsure' to any question, you will need to take the form to a medical practitioner to complete.
Scan your completed self assessment and email it to servicesa@sa.gov.au.
All pages including the front page of the self assessment must be scanned.
