You or your medical practitioner must report a medical condition that could affect your ability to drive. This can be done when:
- you apply for a licence
- you renew your licence
- a condition is diagnosed.
In some cases you may be required to pass a practical driving test too.
How to report

Complete the form
Download and print the correct form for your licence class:

- MR713 Certificate of fitness for heavy vehicle and commercial drivers (848.2 KB PDF)
You can also get one from a Service SA centre.
Fill in Section 1 of the form.
Your medical practitioner must complete the rest of the form. For some medical conditions, you'll need a specialist letter.
Alternatively, you can contact Service SA on 13 10 84 to notify us of your condition. We'll then post you a certificate to have completed by your doctor.

Medical conditions
Many medical conditions can affect the ability to drive safely — such as:
- blackouts
- cardiovascular conditions
- heart disease
- diabetes controlled by insulin or tablet
- hearing loss
- neurological conditions such as epilepsy, seizures, dementia and head injury
- psychiatric conditions
- sleep disorders
- arthritis
- vision and eye disorders.
