You or your medical practitioner must report a medical condition that could affect your ability to drive. This can be done when:

  • you apply for a licence
  • you renew your licence
  • a condition is diagnosed.

In some cases you may be required to pass a practical driving test too.

How to report

  1. Step 1

    Complete the form

    Download and print the correct form for your licence class:

    You can also get one from a Service SA centre.

    Fill in Section 1 of the form.

    Your medical practitioner must complete the rest of the form. For some medical conditions, you'll need a specialist letter.

    Alternatively, you can contact Service SA on 13 10 84 to notify us of your condition. We'll then post you a certificate to have completed by your doctor.

  2. Step 2

    Submit the form

    In person

    Submit the form in person at a Service SA centre.

    Post

    Mail the form to:

Medical conditions

Many medical conditions can affect the ability to drive safely — such as:

  • blackouts
  • cardiovascular conditions
  • heart disease
  • diabetes controlled by insulin or tablet
  • hearing loss
  • neurological conditions such as epilepsy, seizures, dementia and head injury
  • psychiatric conditions
  • sleep disorders
  • arthritis
  • vision and eye disorders.
