Remove an eyesight condition from your licence

If you or your medical practitioner notifies Service SA that you need to wear corrective lenses while driving, an 'S' condition will be recorded on your licence.

To remove the condition, your medical practitioner or optometrist are required to complete an eyesight certificate.

  1. Step 1

    Complete the form

    Download and print the form, or get one from a Service SA centre.

    Fill in Section 1 of the form.

    Your medical practitioner or optometrist must complete the eyesight certificate section.

  2. Step 2

    Submit the form

    In person

    Take the form to a Service SA centre.

    Post

    Post the form to:
