Surrender a SA driver's licence voluntarily

If you no longer wish to drive or are no longer fit to drive, you can surrender your licence. You can also surrender a licence on behalf of a person who is deceased.

1. Complete the form

If you are completing this form on behalf of someone who is deceased, you'll also need one of these:

  • a death certificate
  • newspaper death notice
  • letter from a solicitor.

Any refund cheque will be issued in the name of the estate of the deceased.

2. Submit the form

In person

Submit the form with your photo licence in person to a Service SA centre.
