    • newspaper death notice
    • letter from a solicitor.
  2. Step 2

    Submit the form

    In person

Submit the form with your photo licence in person to a Service SA centre.

    Post

Post the form with your photo licence to:

What happens next

The surrender takes effect from the date it's processed.

If a refund applies, you can choose to get your refund by either:

  • bank account - takes 1 to 3 days
  • cheque by post - takes up to 14 days.

If the form was submitted on behalf of someone who is deceased, the refund will be sent as a cheque by post within 14 days and will be issued to the estate of the deceased.

Bank account refund

When you surrender in person or by post

If you choose the bank account refund option, make sure you have a refund bank account set up in mySAGOV.

If you don't set up a refund bank account within 2 days of surrendering, your refund will be sent as a cheque by post within 14 days instead.
