Check if you're eligible
Mobility impairment criteria
You may qualify if you:
- use a mobility aid or device due to a mobility impairment, or
- have a medical condition or impairment that:
- severely limits how quickly you can move, or
- makes it difficult to walk short distances without stopping due to pain, fatigue, or balance issues.
If either of these apply, you may be eligible for a permit.
Cognitive, behavioural or neurological impairment criteria
You may be eligible if you:
- have a significant cognitive, behavioural or neurological condition, and
- need ongoing support from another person to move safely.
This includes conditions such as autism or other intellectual and developmental disabilities. This applies to children or individuals who require supervision from a parent or guardian.
Legally blind criteria
If you are legally blind and travel as a passenger in a vehicle, you can apply for a permit. You do not need to hold a driver’s licence.
There is no age restriction to be issued a disability parking permit.
How to apply
- Step 1
Complete your section on the form
You'll need to complete and sign section 1 of the application for a disability parking permit form (263.3 KB PDF).
A parent, guardian or agent may complete the form if you are unable to.
- Step 2
Take the form to an approved health practitioner
A health practitioner must complete and sign the relevant sections of your application form. They’ll need to explain how your condition affects your ability to move around.
Depending on your condition, the following practitioners are approved to complete the form:
- Mobility impairments (including use of mobility aids): a medical practitioner, specialist, or occupational therapist.
- Vision impairments: a medical practitioner, optometrist, or ophthalmologist.
- Cognitive, behavioural or neurological impairments that result in being unable to mobilise safely: a medical practitioner, specialist, occupational therapist, or psychologist.
- If a Medical Certificate is required on the application, it must be completed by a medical practitioner, specialist or optometrist (if it's an eyesight condition).
- Step 3
Submit the form and pay the fee
You'll be issued either a:
- Temporary permit for up to 12 months, depending on what your health practitioner declares on the form.
- Permanent permit for up to 5 years.

- Permanent is $24

Take the form to a Service SA centre. A third party can submit the form on your behalf if your details haven't changed.
By post
Post form with the correct fee (money order or cheque marked 'not negotiable' and made payable to Department for Infrastructure and Transport) to:

What happens next
Your permit will arrive in the post within 3 weeks (if approved).
If you don't receive it in that time, phone Service SA on 13 10 84.
