Add guardian details to a death record

People who are court-appointed guardians at the time of a child's death can apply to have their name included on the child’s death registration and certificate.

Funeral directors must advise BDM of the guardian's request when they register the child's death, to allow time for the guardian to provide information to BDM.

How to apply

Download and print the form. Fill it out using a blue or black pen.

Guardians need to submit the completed form to BDM along with:

  • a copy of the Youth Court of South Australia court order
  • the guardian's identification (or certified copies).

Submit the form

Applications must be lodged at the Births, Deaths and Marriages Registration office in person or by mail.
