Apply for motor driving instructor licence card replacement

You can request a replacement of your physical motor driving instructor (MDI) licence card if:

  • it is damaged, destroyed or lost
  • you have changed your name
  • you want the 'Elected to Train Adults' condition removed.

Who can apply

To apply for a MDI licence card replacement you must be a current MDI in South Australia.

If you're applying for the 'Elected to Train Adults' condition to be removed from your MDI licence, you will need a completed Working with Children Check.

Apply online

If your driver's licence address is not up to date, update your address before you submit your request.

Resume an application you have already started.

What happens next

You will be notified to pay the required fee and you will have to pay over the phone (by credit card).

You may need to attend the Service SA centre to update your photo prior to having a MDI licence issued.

Once the licence fee is paid, your photo MDI licence will be sent to your driver's licence registered address within three weeks.
