When verifying your ID online, the Document Verification Service (DVS) will ask for the state of issue of your birth certificate.

Certificates issued from the following dates are accepted from each state.

  • ACT: 1930
  • NSW: 1914
  • NT: 1870
  • QLD: 1974
  • SA: 1944
  • TAS: 1970
  • VIC: 1929
  • WA: 1929

The DVS will require information contained in certain fields.

Example South Australian birth certificates are provided below, with mandatory fields marked in red.

SA Birth Certificate 1

Example South Australian birth certificate from 2001 with the following fields marked in red: family name; given name; date of birth; 8-digit registration number; issue date; certificate number.

SA Birth Certificate 2

Example South Australian birth certificate from 1981 with the following fields marked in red: family name; given name; date of birth; 8-digit registration number; issue date.

SA Birth Certificate 3

Example South Australian birth certificate from 1961 with the following fields marked in red: family name; given name; date of birth; 8-digit registration number; issue date.

SA Birth Certificate 4

Example South Australian birth certificate from 1957 with the following fields marked in red: family name; given name; date of birth; 8-digit registration number; issue date.
