Add parent’s details to a birth certificate

The father's details can be added to a birth record if the child was conceived naturally.

The co-parent's or father's details can be added if the child was conceived with the help of a fertilisation procedure, which the father or co-parent consented to, and at the time of conception were either:

  • legally married
  • in a registered relationship or partners in a marriage-like relationship.

How to apply

  1. Step 1

    Complete the form - Add parent's details to a birth record (PDF 162KB).

    Both parents need to sign the form and a Justice of the Peace (JP) or a solicitor must witness both signatures. Find a Justice of the Peace

  2. Step 2

    Attach copies of your supporting evidence:

    • child's original birth certificate, not a photocopy
    • two types of ID for each parent - photocopies must be certified by a JP or solicitor
  3. Step 3

    Post your application to:

    Consumer and Business Services
    GPO Box 1351
    Adelaide SA 5001

A child's name isn't changed when their father's details are added to the certificate.
