Apply for a proof of age card

Who is this for

People aged at least 17 years and 11 months are eligible to apply for a proof of age card.

How to apply


If you have a photographic driver’s licence or learner’s permit, or have had one in the past, you can apply online.


If you have a mySA GOV account your digital proof of age card will be available instantly.

In person

Complete the application for proof of age card (71.0 KB PDF).

Lodge the form at a Service SA customer service centre, with your evidence of identity and payment.

Combined identification documents need to show full name, date of birth, signature and current South Australian residential address.

If you have a photographic driver's licence or learner's permit you may not need to supply full evidence of identity.

What happens next

Allow up to one month for your proof of age card to arrive in the mail.

If you have a mySA GOV account your digital proof of age card will be available instantly and is an electronic form of the physical card.

Additional information

A proof of age card does not expire.

There are no concessions for a proof of age card.

Replacement proof of age cards are issued using the process outlined in the Online or In person tabs above.

You can digitise your proof of age card with the mySA GOV app to access your information online.
