- Your residential address history with Service SA.
- Any postal addresses.
- The dates you updated your address.

Take the completed form to a Service SA centre and pay the fee.

What happens next
If you applied online, your address history report will be available to download from your mySAGOV account.
When you pay the fee at a Service SA centre you will be given a printed report.
