To apply for a cremation permit, the funeral director must:

  • give the family, the executor or the administrator of the deceased estate, the application for a cremation permit form to complete and sign
  • upload the completed form, together with other supporting documents, to BDM.

Use the notes for funeral directors to ensure the doctor has provided all other relevant forms.

The fee is $60.50.

When registering the death of a child, funeral directors must advise Births, Deaths and Marriages (BDM) if the child's court-appointed guardian(s) wish to have their details added to the child's death record. This is to allow time for the guardian to provide information to BDM.
