Other ways to apply for a seniors card

1. Application form

Print and complete the Seniors Card application form (1.4 MB PDF).

Application forms are also available from South Australia Public Libraries.
Find your nearest library

2. Proof of age

You'll need a document that proves how old you are - for example:

  • birth certificate
  • drivers licence
  • passport.

3. Sign and send

You must sign the form in front of someone who is over 18 years of age and not related to you. They must sight your birth certificate, driver's licence, passport or a similar document as proof of your date of birth.
