Health care plans and forms

Health care plans specify the arrangements and staff training necessary to help children with health conditions to attend school or preschool. You should tell staff when you enrol your child if individual health support is needed and advise them if health care needs change.

Health care plans

You will be asked to provide a health care plan written by the treating health professional to ensure appropriate arrangements are made to support your child. The plan should document your child's emergency and routine health and personal care needs. The school or preschool can help you choose the forms you need.
