Apply for a proof of age card

Eligibility

You must be 18 years of age or older to be eligible for a proof of age card.

Before you start

To apply, you'll need to provide evidence of identity. This is a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity. A payment is due upon application.

How to apply

1. Application form

Complete the form application for proof of age card 112.9 KB.

2. Lodgement

Return the form to a Service SA customer service centre, with identity documents and payment. You will be photographed at the time of application.

Allow up to a month for your proof of age card to arrive in the mail.

3. On-line application

If you already have a driver's licence or learner's permit and your photo was taken less than 30 years ago, you can apply for a proof of age card via EzyReg. Your date of birth must be entered.

There is a fee for Proof of Age cards.

Replacing a proof of age card

If your photo is less than 30 years old you can apply for a replacement proof of age card online; or you can make a new application at a Service SA customer service centre.

If you attend a Service SA customer service centre you will need to provide evidence of identity.  You may not need to supply full evidence of identity if you already have a photographic learner's permit or driver's licence.

There is a fee for the issue, replacement of a proof of age card.

Allow up to a month for your card to arrive in the mail.

A proof of age card does not expire.

Change of address

If you change your address you can you can apply for a change of address sticker online via EzyReg.
