Apply for a proof of age card

Eligibility

You must be 18 years of age or older to be eligible for a proof of age card.

Before you start

To apply, you'll need to provide evidence of identity. This is a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity. A payment is due upon application.

How to apply

1. Application form

Complete the form application for proof of age card.

2. Lodgement

Return the form to a Service SA customer service centre, with identity documents and payment. You will be photographed at the time of application.

Allow up to a month for your proof of age card to arrive in the mail.

Renewing a proof of age card

There are two ways to apply for a replacement proof of age card:

  1. apply online for a replacement card (provided your photo is valid)
  2. visit a Service SA customer service centre, with:
    • evidence of identity - a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity
    • payment.

Allow up to a month for your replacement card to arrive in the mail.
