Register a birth

The arrival of your baby needs to be registered with Consumer and Business Services (CBS) within 60 days of their birth.

There is no fee to register a birth in South Australia.

What you'll need

You'll need to provide these details:

  • date and place of your child's birth
  • how much your baby weighed at birth (see Birth Registration Statement)
  • doctor or midwife (see Birth Registration Statement)
  • birth mother
  • father or co-parent
  • the parents' marriage (if appropriate)
  • previous children.

Online

Apply on the Consumer and Business Services website.

Register a birth

Once completed, CBS will email you the completed form to sign and return.

Post

1. Application form

Complete and sign the form given to you by the hospital or your midwife.

2. Supporting evidence

Section 10C Declaration must also be filled out if your child was conceived with the help of a fertilisation procedure and you haven't lived together as domestic partners for at least three out of four years.
