This form is used for persons who wish to install an industrial pump in their premises where the supply of motor fuel to the occupier of the premises is principally for that occupier's own use and not for the retail sale of motor fuel. An Industrial Pump is defined as a pump connected to a bulk tank having a capacity of not less than 2001 litres. The amount of motor fuel supplied for use in that pump must not be less than 6800 litres per month. The legislation provides for certain exemptions to these conditions. The application must be accompanied by a Statutory Declaration.

This SafeWork SA form is to be completed within seven days of the installation of a tank (whether above ground or underground and greater than 450 litres capacity) that has been approved by SafeWork SA for the storage of a dangerous substance or petroleum product.

This form is used by existing Petroleum Site Licence holders who wish to object to the proposal being considered by the Petroleum Products Retail Outlets Board. Those located within 3kms of the proposed site will be contacted by the Secretary and requested to provide this information should they wish to formally object to the proposal. This form requests details of volumes of motor fuel sold within the previous three years from an objectors petroleum site premises.

This licence is required to sell motor spirit, lubricants and other permitted goods on any day after closing time and on Sundays and public holidays where the majority of sales are for motor spirit and lubricants.
