2/06/2009 5:55:22 PM [DI-09] To comply with the Births, Deaths and Marriages' Access Policy, all death certificates must be applied for by the spouse (including defacto), a parent or a child of the deceased person. Written authorisation from the above mentioned persons applies to other applicants. Applications can be submitted online via the OCBA website or order forms can be downloaded in PDF format. Forms are also available from South Australian Bill Paying Post Offices, or from the Births, Deaths and Marriages Registration Office. Current identification (ie not expired and showing current address) is required for all applications.

2/06/2009 5:55:27 PM [07] Any death that occurs in South Australia must be registered with the Registration Office, of the Office of Business and Consumer Affairs. In most cases, the Funeral Director will complete a Death Registration Statement and forward to the Registration Office on behalf of the family within 7 days after the funeral.
