Complete this document (one per child) and deliver or post to the address provided within 60 days of the child's birth. Lodgement of this document is important as it enables a birth certificate to be provided when required. Ensure the information is accurate so as to avoid future problems when applying for a birth certificate. There is no fee to lodge a Birth Registration Statement however, there is a prescribed fee for either a standard or commemorative birth certificate.
