2/06/2009 5:55:27 PM [07]A Birth Registration Statement document should be provided to you at the time of your child's birth by the hospital. Complete this document (one per child) and deliver or post to the address provided within 60 days of the child's birth. Lodgement of this document is important as it enables a birth certificate to be provided when required. Ensure the information is accurate so as to avoid future problems when applying for a birth certificate. There is no fee to lodge a Birth Registration Statement however, there is a prescribed fee for either a standard or commemorative birth certificate.
