At least three documents are required to provide evidence of identity.
At least one document must be provided from both
category A (evidence of existence) and
category B (evidence of use of identity). Your signature, date of birth and current address must be on at least one piece of evidence.
If none of the documents from category A or B shows your current address, you must provide a fourth document showing your
current residential address.
Documents must be originals and issued within the allowable time. Photocopies, extracts or commemorative certificates are not acceptable.
Category A - evidence of existence
The following documents are acceptable:
an Australian full birth certificate showing parental details (a Commemorative Certificate, certified copy or extract is not acceptable)
an overseas birth certificate showing parental details provided a passport or an official Australian travel document is also shown
a current Australian passport or one that has expired within the last two years
a current overseas passport (with or without visa) or an overseas passport that has expired within the last two years (provided it is accompanied by a current Australian visa)
Note: Electronic visas are not acceptable as Evidence of Identity
an Australian naturalisation or citizenship document or immigration papers issued by the Department of Immigration and Citizenship (DIAC). Note: A person under the age of 18 who is naturalised along with their parents may not necessarily be issued with an individual citizenship document; they may instead appear on the parents’ citizenship papers
an “Extract from Register of Citizenship by Descent” (issued by an Australian embassy where the parent(s) are Australian-born)
Note: the term “Extract” is the official title and is not an extract in the same sense as an extract from an Australian full birth certificate
Department of Immigration and Citizenship (DIAC) “Certificate of Evidence of Residence Status
Department of Immigration and Citizenship (DIAC) travel document (valid up to five years after issue). Note: A person under the age of 18 who immigrates to Australia with his/her parent(s) will generally be included on the immigration paperwork of their parent(s)
photograph and/or signature stored on the photographic licence database
Australian Defence Force photo identity card (excluding civilian staff and family)
South Australian or Federal Police Officer photo identity card
New South Wales photo card (issued after 14 December 2005)
South Australian proof of age card (issued after 9 October 2006)
Australian photo driver’s licence or learner’s permit (current or expired up to two years). The licence / permit must display a card number and not be reported as lost, stolen or destroyed.
Category B – evidence of use of identity
The following documents are acceptable:
a current Medicare card, Pensioner Concession Card, Department of Veterans' Affairs entitlement card or any other current entitlement card issued by the Australian (i.e. Federal) Government
a telephone, gas or electricity bill up to six months old
a water rates, council rates or land valuation notice up to twelve months old
an electoral enrolment card or other evidence of enrolment up to two years old
student identity document from an educational institution with photograph and/or signature (current or expired up to twelve months)
Australian issued security guard or crowd controller licence showing photograph
occupational licence (OCBA licence) showing photograph
an armed services discharge document up to twelve months old
Australian issued firearms licence showing photograph
a current credit card, that shows the embossed name and signature, or account card from a bank, building society or credit union, or a passbook or account statement up to one year old.
Note: ‘Screen dumps’ from financial institutions are not acceptable, with or without financial institution branch stamps.

Evidence of residential address
A person must be a resident of South Australia to be eligible for the issue of a learner’s permit or driver’s licence. Evidence of a South Australian address is required when a person applies for a driver’s licence or learner’s permit.
A person is only required to supply one of the documents listed below if their residential address has not been established by a Category A or B document:
current South Australian vehicle registration certificate
South Australian driver’s licence or vehicle registration renewal for the coming period
a contract of sale, lease arrangement or rental receipt for his/her place of residence up to two years old
last or current financial year Australian Taxation Office tax assessment (note: client must have blanked out their Tax File Number (TFN) prior to presentation)
certificate or statement of enrolment from an educational institution (up to one year old)
a letter from the principal of a recognised educational institution verifying the person’s address (up to one year old)
a statement or letter from a bank, building society or credit union (up to one year old) which verifies the person’s residential address.
Note: ‘Screen dumps’ from financial institutions are not acceptable, with or without financial institution branch stamps.

Evidence of change of name
If you have changed your name, through marriage, divorce or through an Australian Registrar of Births, Deaths and Marriages (BDM), you must provide documents that prove the link between your current and previous names.
The following documents are acceptable:
And one of the following as a 'linking' document:
- birth certificate with an endorsement from an Australian BDM.
- marriage certificate issued by an Australian BDM (Commemorative Certificates issued by a religious leader or celebrant are not acceptable).
- change of name certificate issued by an Australian BDM.
- deed poll certificate (previously issued by Lands Title Registration office).
- divorce papers Decree Nisi or Absolute (indicating the name being reverted to).

The following will be accepted as evidence of residential address for a body corporate premises:
- contract of purchase, current lease or rental document for relevant premises
- current vehicle registration certificate
- vehicle registration renewal notice
- current council rates notice
- current land tax valuation notice
- Australian Taxation Office assessment for the last or current
- financial year (note: you must black out your tax file number before presenting).

If the garaging address of the vehicle is different from the residential address for the body corporate, evidence of where the vehicle is garaged is required:
- certificate of title
- contract of purchase, current lease or rental document
- Australian Taxation Office assessment notice in relation to the premises for the
- last or current financial year (note: you must black out your tax file number before presenting)
- current council rates notice
- utility account up to 6 months old.
