You must be 18 years of age or older to be eligible for a proof of age card.
To apply:
Step 1: Complete the form
application for proof of age card.
Step 2: Return it to a Service SA
customer service centre, with the following:
- evidence of identity - a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity.
- payment.
Allow up to a month for your proof of age card to arrive in the mail.
Renewing a proof of age card
There are two ways to apply for a replacement proof of age card:
- apply online for a replacement card (provided your photo is valid)
- visit a Service SA customer service centre, with:
- evidence of identity - a minimum of three documents that verify your identity, age, signature and residential address. If you have a photographic learner's permit or licence you may not need to supply full evidence of identity
- payment.
Allow up to a month for your replacement card to arrive in the mail.
