The Aged Care Assessment Team (ACAT) provides advice, and where necessary, assessment to older people with complex care needs on suitable residential or community care options.
ACATs assess and approve older people for Commonwealth subsidised aged care services such as:
- Community based support: Community Aged Care Package (CACP) or Extended Aged Care Package (EACH) or EACH Dementia
- Residential care: low level care or high level care)
- Residential respite care: low level respite care or high level respite care
- Post-hospital support: Transition Care Program.
An ACAT assessment is not required for HACC services - eg Domiciliary Care, councils and other local HACC programs.
Referrals
Referrals can be made:
- directly by the older person needing assessment
- or by their family, carer or health professional (with consent from the older person).
