Entry to the South Australian Carer Recognition Awards is open to all businesses, not for profit organisations, government agencies and general practices operating in South Australia. For more information see frequently asked questions.

Category 2 - Outstanding initiative for carers as employees 

How to make a nomination

Nominations are open 1 May until 13 July 2012.

An initiative can be nominated in more than one category. Multiple awards can be won by one organisation if each nomination details a different initiative. All nominations are subject to the terms and conditions (PDF 31KB) (Word 87KB).

Make a nomination in three ways:

  1. complete an online application (PDF 315KB) 
  2. print and complete the application form (PDF 315KB) and post to:

  3. complete a reply paid nomination form (available from some general practice waiting rooms).
