What happens after you make a referral?

You can apply for services from Disability Services in three ways:
- phone the Disability Information Service on 1300 786 117
- visit or phone your closest Disability Services office
- request a referral pack, which you will need to complete and return by post, fax or email.

Referral forms
To make a referral you will need to complete two forms:
- Disability Services referral form (Word 284KB)
- Consent for eligibility and intake assessment form (available from us).
We require a signed copy of the consent form even if the original referral has been completed by telephone.
We will also ask you to complete additional forms for:
- children aged under 5 years
- people aged over 5 years with a suspected intellectual disability or an autism spectrum disorder.

Other paperwork to send with your referral
To help us to process your referral as quickly as possible, we will ask you to include any reports that provide information about your disability and needs. These may include:
- medical reports
- hospital discharge summaries
- therapy reports
- psychological or Guidance Officer reports.

What happens after you make a referral?
- A letter will be sent confirming that we have received your referral.
- Your referral will be assessed by the Intake Team.
To make a proper assessment, we may contact you and arrange to meet with you at home, in hospital or at a Disability Services office.
We may also request additional assessments which we will discuss with you.
- A letter will be sent advising you of the decision about whether the person being referred is eligible for services.
This letter will be sent to the person who has been referred and, if required, to a parent or the guardian of that person.
If you are eligible for support from Disability Services, a member of the team in your area will make contact with you as soon as possible. There may be a waiting period for some support.

What if you are not eligible for support from Disability Services?
